Vital Records
Records of births and deaths that have occurred in
Poweshiek County are maintained in the Poweshiek County Recorder’s
office. Marriage records are maintained in the county where the
license to marry was obtained.
We do not have single parent births prior to July of 1995, birth
records if you were adopted; or birth, death and marriage records
between the years 1921 thru 1941. For the records we do not have,
you may contact the Iowa Department of Public Health at (515)
281-4944.
All applications for certified copies of vital
records must be in writing and completely identify the record, as
well as satisfactorily establish entitlement to the record being
requested. Entitled persons include the person named on the record
or that person’s spouse, children, legal parents, grandparents,
grandchildren, siblings, or legal representative or guardian. A
photo ID including signature must be provided. Applicants must be 18
or older. Requests must include the applicant’s current
government-issued photo identification, except if by mail, a clear
photocopy of the ID, and the applicant’s signature signed in front
of a notary public or in the presence of an Iowa Registrar of Vital
Records.
A non-refundable $15 fee is required to search for a record and
includes one certified copy if the record is located. Each
additional copy of the same record is $15.
Birth, Death or Marriage Records
To obtain a certified copy of a birth certificate, death certificate
or marriage record you can write to us or come into the office. We
have Poweshiek County records only.
